On February 22, 2025, the Poltava Regional Clinical Cardiology Dispensary successfully performed its first transcatheter aortic valve implantation (TAVI). This revolutionary procedure allows for valve replacement without open-heart surgery. The operation was carried out by vascular surgeon Yevhen Liulka alongside a team of experienced specialists.

Interview with Yevhen Liulka

— Dr. Yevhen, how did you prepare for your first transcatheter aortic valve implantation surgery?

This journey took several years. The preparation process was quite extensive. It all began back in 2021, when our team attended the first training courses and simulation workshops. However, the full-scale invasion temporarily halted our progress. Only in late 2024 were we able to resume our efforts. Our doctors underwent a series of trainings—both online and on simulators—and traineeship at the Amosov Institute of Cardiovascular Surgery. Thanks to this extensive preparation, we were able to perform the surgery at the highest level.

— This procedure is not suitable for every patient. What factors influenced your decision to select this particular patient?

Aortic valve disease is a common condition. Most patients with stenosis or insufficiency undergo traditional open-heart surgery. However, for some individuals, such an operation is too risky—particularly elderly patients or those with serious comorbidities. This minimally invasive procedure was specifically designed for them.

— What challenges did you face during the surgery?

Our first procedure went smoothly, without complications. We had prepared for every possible scenario. Our team included not only vascular surgeons but also cardiac surgeons, who were ready to step in if needed. Additionally, we received assistance from experienced specialists at the Amosov Institute. Thanks to this support, the surgery proceeded exactly as planned.

— Did the surgery meet your expectations?

Yes, the reality exceeded expectations. The endovascular stage of the operation lasted about one hour, while the entire process—including preparation and patient stabilization—took three hours. Everything went according to plan, and we achieved the desired outcome.

— How is the postoperative recovery going?

The patient is doing exceptionally well and is already preparing for discharge. After surgery, he spent a day and a half in intensive care, under close monitoring. Now, he is walking, standing, and taking care of himself independently.

This surgery is a major milestone for medicine in the Poltava region. Local residents now have access to modern, less traumatic treatment options for heart disease. And this is just the beginning!
